The person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection, by not complying with the food code and by having multiple priority item violations during this inspection. Food employees are unable to properly wash their hands due to the hot water being turned off at the hand-wash sink in the food preparation area; repeat violation. An open employee’s beverage container was on a food preparation table and on a shelf with food; repeat violation. Assorted food in the reach-in cooler stored open with no covering; repeat violation. Eggs held at 72 F at the make line, rather than 41 F or below as required. Quiche, ham, meatloaf, roast beef and cheese were held at 50 F in the drawer-style cabinet beneath the flat grill, rather than 41 F or below as required. Temperature control for safety foods are not permitted to be stored in this cooling unit until it maintains a temperature of 41 F or colder; repeat violation. Refrigerated, ready-to-eat, time/temperature control for safety food, (spaghetti, chicken fritter mix and other assorted food) prepared in the food facility and held for more than 24 hours in the walk-in cooler is not being date-marked; repeat violation. Barbecue meat, ham and other foods, refrigerated, ready-to-eat, time/temperature control for safety foods in the walk-in cooler were date-marked by the facility but were beyond the seven-day use- or sell-by date and require discarding; repeat violation. Time in lieu of temperature being used for ready-to-eat, potentially hazardous foods (raw eggs) without written procedures or documentation to verify disposition of food. Prepackaged cookies, whoopie pies, brownies and muffins are not labeled properly with the name of product, ingredient statement, net weight, distributed by statement; repeat violation. Prepackaged cookies, whoopie pies, brownies and muffins are not labeled to clearly indicate any “Big 9” allergen ingredients and/or the allergen warning statement; repeat violation. Old, hardened food residue on the slicer blade and inner rim of the slicer; repeat violation. Black static dust on the ceiling above the make station. Nonfood contact surfaces on the floor behind the flat grills and fryers on the floor under the hot-holding area, handles, on top of the two-burner stove, floor beneath all equipment in the food preparation area not cleaned at a frequency to preclude accumulation of dirt and soil; repeat violation. The hand-wash sink in the food preparation area was blocked by a cart and not accessible at all times for employee use. Plumbing system not maintained in good repair: water leaking at the profusely at the faucet of the hand-wash sink in the food preparation area between the walk-in cooler and freezer. The floor throughout the back food preparation area and the dishwashing area is torn, bubbling and not smooth and easily cleanable. Lights are not shielded or shatterproof over the mechanical dishwasher.

Black, standing, stagnant water on the floor beneath the pre-wash area in the dishwashing room creating a putrid odor. The entire floor/wall juncture in the dishwashing room has black, putrid, organic material on it.
