OSHA has proposed $159,117 in penalties to All Phase Roofing and issued citations alleging three repeat and three serious violations after a group of employees was observed installing a roof membrane in Boca Raton, Fla., without fall protection, the Labor Department said on Friday.
All Phase Roofing’s alleged violations included not using fall protection, not having a competent person inspect the work site to make sure employees had proper safety equipment, and allowing employees to perform roofing work before safety hazard and fall protection training, according to the DOL.
