OSHA cites plumbing contractor for fatal trench collapse in Buffalo Grove

Federal investigators have determined that the ditch in Buffalo Grove where a man from Elmwood Park died after being buried in December did not have the required collapse protection.

Nikodem Zaremba, 27, was 7 feet below the surface repairing a residential water line on the 1000 block of Aspen Drive when the moat collapsed.

According to a press release issued this month, the Occupational Safety and Health Administration found that Rooter Solutions Inc., a plumbing company based in Burr Ridge, had not installed collapse protection to prevent the collapse and did not ensure the use of the required headgear.

Rooter Solutions faces a $35,940 fine.
